New Office Policy effective January 2017:
Annual Administrative Fee for Non-Covered Services**
$75 per child; maximum $200 per family of three or more children
**Non-covered services are those services that WPA performs that are not
paid by insurance companies. Typically, these "non-covered" services
require considerable time to do outside of the office visit. For example:
- Phone conversations to insurance companies and HR depts. regarding
insurance eligibility and billing questions;
- Resubmitting and appealing denied claims to insurance companies;
- Coordination of care with specialists/ ER doctors, pharmacies, laboratories
or radiology departments, as a follow up to an office visit;
- Completing medication refill requests without an office visit;
- Case management planning for complex medical conditions or behavioral
concerns involving collaborative discussions between parents/care-givers,
teachers, school counselors and/or mental health professionals;
- Drafting letters for patients for a variety of reasons;
- Medical literature reviews on complex conditions;
- Miscellaneous extra time spent on a case by case basis.

admin fee will allow WPA to continue to provide quality care to your

Co-Payments
We request copayments at the time of service. For any service not covered by your insurance policy, you will be responsible for the cost of treatment.
Please remember to bring your insurance card and photo id to every appointment
School Forms And Referrals
Please allow two weeks for the completion of school forms and referrals.
Fees
- Missed appointments: $50
Kindly give 24 hours notice if unable to keep your appointment.
- Returned checks: $35
- After Hours Calls: $20
- Completion of basic school/health forms:
- School/ camp forms: $30
*Please allow 2 weeks for completion of all forms.
- Copying medical records: $0.76 per page
